Please arrive 15 minutes prior to your appointment time; or, if you would prefer to download and fill out the attached forms, you may arrive 10 minutes before your first appointment.  In addition to your completed patient information form, please bring your driver’s license, insurance card and the physical therapy prescription from your physician.
